Applications and Information

If your child is selected, we will need the following items:

2. Waiver Form- http://texasladyheat.com/texasladyheatwaiverform.doc
3. Commitment Letter- http://texasladyheat.com/texasladyheatplayercommitmentform.doc
4. Copy of your birth certificate & Picture of child (for future tournaments, in case the age of child is challenged).
5. $140 Registration Fee (check made out to the Texas Lady Heat).
